Functional Medicine Consultation Online/Video ($299)

• 90 minute functional medicine consultation

• Review of outside medical records and labs

• Review of medications and supplements

• Review of medical and personal history

• Review of health assessment questionnaire

• Preliminary recommendations

• Ordering of comprehensive lab work-up (includes recommendations; labs at an additional cost)​

You must complete the comprehensive health questionnaire at least 72 hours prior to your consultation. This is essential in order to give the provider time to review your medical history. Failure to complete the questionnaire will result in a cancellation of your appointment.

Follow-up Visit Online/Video ($99)​

• Follow-up visits are typically 30-40 minutes depending on complexity and your needs. We will review your lab work and make updates to your personalized care plan
